Insurance, Terms, & Conditions

For legal purposes, our standard rate for service is $200.00/hr.

It is our business policy to take payment at the time services are rendered and as such,

most payments qualify for a “Time of Service”(TOS) discount of $100/hour.

The TOS discount may also be known as a bookkeeping/prompt-pay/self-pay discount and is not prohibited under WA State law.

To simplify pricing information for the general public, the prices listed on this page include the TOS discount.

We accept most insurances including auto and personal injury claims!

If you do not see your insurance company below it may be a smaller branch of one of these larger companies and there is a good chance we will still accept it.  Just ask our billing office in advance at billing@nwroots.com

Having insurance does not guarantee coverage of massage therapy - we ask that you contact us so that we can then call on your behalf. 

Please note that most insurance only covers up to one hour per day and requires a prescription.

If your insurance company is not listed, no problem, we will bill them for you! Please just understand there is a process to this and we still need a referral from your provider before or at your first appointment. We are happy to assist, just give us a call or send us an E-mail.

Important Information Regarding Insurance Claims:
  • Please check with your insurance company regarding your benefits prior to your visit. If we bill your insurance company and they deny your claim, then you are required to pay for services out of pocket.

  • In order to bill your insurance company for medical massage, you must have a prescription from your doctor or another medical professional that includes: the date issued, diagnosis, and ICD-10 code, plus the recommended frequency and duration of treatment.

  • If you do not have a prescription, you will be required to pay out of pocket for the visit. If you obtain a prescription after the service has been performed and the prescription is back-dated, we can give you a refund after we receive payment from your insurance company.

  • Any balances not covered by insurance are due within 60 days of the initial billing unless other arrangements have been made with our billing department.

  • Please arrive around 15 minutes early for your visit to complete the necessary paperwork, including the 'Client Intake" forms sent to your email. Please fill it out prior to your appointment
